Flu Vaccination – Missed Consent Opportunity

Dear Parents/Carers, 

 

Please find attached a letter from the School Age Immunisation Team regarding the Flu vaccination. This is for families who have not yet submitted an E-consent form.

 

The portal has now been re-opened to allow you to submit consent if you still wish for your child to receive the vaccination. Once submitted, you will receive an email from Cinnamon Digital with a link to book a clinic appointment.
